The service is amazing but some areas were not answered. I could not get the text form fields (as text inputs). that was the problem. please help get me the revised code.

the form input fields do not work, in spite of using the correct # tags as mentioned.

As I can see, you have tagged text layers with #in_text tag. However, rectangle layers should have been tagged instead. If you tag rectangle layer with #in_text then service will automatically find a text placeholder for it.
